Subject: [PATCH] chore(): add information to issue template

The following information was added:

* removed the v1 checkbox, left link to v1 repo
* added link for Ionic Pro issues
* more clearly redirect How To type questions to the public forums
* suggest sample code in following order: GitHub, StackBlitz, Plunker
