This patch finalizes the C++ conversion of the ui-out subsystem, by turning the ui_out and ui_out_impl structures into a single class hierarchy. ui_out functions are turned into virtual methods of that new class, so as a result there are a lot of call sites to update. In the previous version of the patchset, there were separate ui_out and ui_out_impl classes, but it wasn't really useful and added boilerplate. In this version there is simply an ui_out base class that is extended for CLI, TUI and MI.
