61baf725ec update copyright year range in GDB files 
This applies the second part of GDB's End of Year Procedure, which
updates the copyright year range in all of GDB's files.

462f517e50 Do not include defs.h or server.h in any header file 
This commit removes all inclusions of defs.h and server.h from header
files.

28e7fd6234 Update years in copyright notice for the GDB files. 
Two modifications:
  1. The addition of 2013 to the copyright year range for every file;
  2. The use of a single year range, instead of potentially multiple
     year ranges, as approved by the FSF.
