78e5999d37 remove use_windows 
Nothing actually uses use_windows, not even Insight.  So, this patch
removes it.

0e9f083f4c remove gdb_string.h 
This removes gdb_string.h.  This patch is purely mechanical.  I
created it by running the two commands:

    git rm common/gdb_string.h
    perl -pi -e's/"gdb_string.h"/<string.h>/;'  *.[chyl] */*.[chyl]

28e7fd6234 Update years in copyright notice for the GDB files. 
Two modifications:
  1. The addition of 2013 to the copyright year range for every file;
  2. The use of a single year range, instead of potentially multiple
     year ranges, as approved by the FSF.
