TRUE/FALSE simplification

There is really no need to write code like "foo != 0 ? TRUE : FALSE"
unless we had stupidly defined FALSE as something other than 0 or TRUE
as something other than 1. The simpler "foo != 0" does just as well.
Similarly "(condition == TRUE)" or "(condition == FALSE) can be
simplified to "(condition)" and "(!condition)" respectively.
I'll note that there is reason to use "integer_expression != 0" when
assigning a bfd_boolean rather than the simpler "integer_expression",
if you expect the variable to have 0 or 1 value. It's probably even a
good idea to not rely on implicit conversion if bfd_boolean were _Bool.

Inserting with replacement is wrong for some gas hash table uses.
This patch implements an htab_insert that conditionally replaces, and
similarly for str_hash_insert. str_hash_insert with replace=0 is
roughly equivalent to the older hash_insert, and str_hash_insert with
replace=1 to the older hash_jam, but return values are different. I
found it useful to know whether the slot was occupied prior to
inserting/replacing. I've also reinstated the fatal errors on messing
up opcode tables with duplicates.

Fix all unexpected failures in gas testsuite for pdp11-aout.
...
These failures were caused by the PDP11's mix of little-endian octets
in shorts but shorts in big endian order for long or quad so regexps
did not match. Also tests used addresses as values in .long which
required BRD_RELOC_32 that was not implemented.
* gas/config/tc-pdp11.c (md_number_to_chars): Implement .quad
* gas/testsuite/gas/all/gas.exp: Select alternate test scripts for
pdp11, skip octa test completely.
* gas/testsuite/gas/all/eqv-dot-pdp11.s: Identical to eqv-dot.s
* gas/testsuite/gas/all/eqv-dot-pdp11.d: Match different octet order.
* gas/testsuite/gas/all/cond-pdp11.l: Match different octet order.
* bfd/pdp11.c: Implement BRD_RELOC_32 to relocate the low 16 bits of
addreses in .long (used in testsuites) and .stab values.

This allows pdp11 to handle 32-bit fixups that can be applied by gas,
and improves the error message emitted when the required reloc isn't
available.

Teach a few targets to resolve BFD_RELOC_8
...
and tidy "forward" test. I've removed some checks in d30v
md_apply_fix that have no business being there. Any symbol problems
will be caught later in tc_gen_reloc, and overflow checking is done in
gas/write.c.

make md_parse_option () take a const char *
...
This is mostly just adding const in many places, however there are a couple
interesting things. We need to add casts in tc-s390.c and tc-cris.c because
they have functions that assign to input_line_pointer an argument that
sometimes comes from md_parse_option. Presumably this is safe because those
targets never pass literals to md_parse_option (), but this code should
probably be improved in the future. Also xtensa passes the argument to strtoll
which is a rather odd function, it takes a const char * as argument and returns
a pointer into that string as a char * through an out argument, but we can work
around that by adding more variables.

tc-pdp11.c: remove useless code

if the condition is true then we know that str already points to a'\0' in the
string passed to the function. Since we know the latter part of the function
doesn't modify that string, and str already points to a null byte there's no
point in changing str to point to a literal empty string.

(parse_op): Reject attempts to refer to float regs.
(parse_fop): New function, like parse_op but for float operand.
(md_assemble): Add cases to parse float operands. Also fix
IMM3, IMM6, IMM8 cases to pick up the operand from the right spot.
